Integrating care systems so that no smoker falls between the gaps: panel discussion

A discussion around exploring  what integrating care systems actually mean  and promise to deliver so that no smoker fall within the gaps in transfer of care.

  • Jon Foster – ‘A Breath of Fresh Air’ Research into conveying the importance of giving Very Brief Advice within Primary Care  – How Good are GP’s?
  • Dr Roger Henderson: A GP specialising in smoking cessation – what conservations and support should be happening with patients pre and post hospital admissions, patients with SMI and with patients who don’t want to or can’t access commissioned smoke stop services.
  • Darush Attar-Zadeh: Primary Care Pharmacist specializing in smoking cessation – covering Varenicline and NRT discharge from hospital discussions as part of an ICS.
  • Anne Joshua: What is the Pharmacy Integration Fund pilot work and early learns so far.
